HMIS Report Card and Services Report (10/1/13 - 7/31/14)

Our most recent HMIS Report Card is posted on our hmisadmin.com site (or you may click this link to access directly), and attached is our most recent Services Report (both cover 10/1/13 - 7/31/14). Please view the concerns below.  I will coordinate with John and Tony for an on-site half day HMIS workshop (w/lunch provided, of course).  In the meantime, please don't hesitate to contact me at any time with any questions.  Thank you! We are nearing the end of SSVF Year 3, so we must ensure all data is accurate and timely.
