DHCD: PPE Needs Survey for Homeless Service Providers

DHCD PPE Needs Survey
To our valued partners:
In response to national and statewide efforts to contain the spread of Coronavirus (COVID-19), we are inviting you to complete this survey to enable us to provide support to providers in need of Personal Protective Equipment (PPE) for shelters, street outreach and other critical services to vulnerable populations. Presently the state of Maryland is working hard to address PPE shortages to protect our essential community providers. While there are national shortages of PPE and we cannot guarantee that we will be able to meet all needs, we are collecting information to assist in triaging provider needs and providing PPE where it is needed the most as shortages are addressed and supplies become available.

Please take a brief moment to complete this survey to let us know your needs and allow the state to assist you in the event that supplies of PPE are secured and made available for distribution. Please also share with your subgrantees and community partners who may be in need of PPE at their locations. Please complete this survey by 12pm on Wednesday 3/25/20.
